This form is for alumni and current athletic students. Current students should use their Naviance account for official transcript requests.

Athletic students under 18 must fill out and turn in a hard copy of the below transcript release waiver form.

The cost of transcripts is $5 payable to Duchesne High School. Please send payment to 2550 Elm Street, St. Charles MO 63301.

Transcript fee may also be paid via credit card by calling the front office at 636.946.6767.

Transcripts will not be sent until payment is received.

Current students should use their Naviance account for official transcripts.

Current students who are athletes do not have to pay the $5 fee.
